How to create an order form with online payment

If you sell products or services, an order form that also takes payment removes a whole step for your customers — no separate invoice, no follow-up, no chasing. Here's how to build one in minutes.

1. Start from the Order Form template

In Formspry, open Templates and pick Product Order Form (or Online Payment Form). It already has the fields most orders need — contact details, product, quantity, delivery address and a payment field. Tweak the products and prices to match your catalogue.

2. Add your products and price

Use a dropdown for the product list and set the payment amount on the Payment field. You can add quantity, size or variant fields and a notes box for special instructions.

3. Collect delivery details

Add address and phone fields so you have everything you need to fulfil the order. Mark the essentials as required so nothing is missed.

4. Get paid and confirm

When the customer submits, they pay securely via your connected Razorpay account and the order lands in your dashboard. They can request a GST invoice or receipt on the spot, emailed as a PDF in your business's name.
