How to make a consent form with an e-signature

Clinics, salons, coaches, event organisers and service businesses all need signed consent. Doing it on paper is slow. A digital consent form captures the same signature in seconds.

1. Lay out the consent text

Use a Paragraph block for the consent or waiver wording, then add a required checkbox like 'I have read and agree' so intent is explicit.

2. Add the signature field

Drag the Signature field onto the form. The person signs with a mouse or finger, and the signature is stored with the submission, the timestamp and everything else they entered.

3. Keep a clean, exportable record
